Program Guidelines

• Our plan is not an insurance plan or a qualified plan under the Affordable Care Act.
• The membership plan includes a 7-day money back guarantee. You can receive a full refund up to 7 days after purchase if no services or discounts have been used.
• Payment for treatments not included in the plan is due at the time or service.
• Perio Plan DOES NOT include initial scaling (20% discount)
• Products, Sleep Apnea Appliance, and KOR whitening are not included.
• Plan discounts do not apply to the following procedures completed at our practice: Orthodontics, such as comprehensive orthodontic treatment and appliance therapy (CDT code D8000-D8999.)
• Our membership plan does not cover procedures completed by specialists outside our practice.
• Plan renews annually on your anniversary date. To cancel, contact us in writing, 30-Days before our renewal date.
